  • PTD Application Revised

    The Industrial Commission has revised its Application for Compensation for Permanent Total Disability (IC-2).  The new application can be found here.  Of note, the Industrial Commission now asks claimants to choose which claims they want considered on their PTD application allowing claimants to exclude minor claims from consideration and avoiding the need for examination on claims no longer causing disability.
